The yellow board at the bottom goes under the gurnery or board the patient is strapped to while the green piston goes over the top position on the sternum and delivers 110 compressions a minute, the CPR standard requirement. The $15,000 Lucas 2 can also be plugged into the ambulance power through and invertor port so it does not rely on the device battery
